Individual counseling is a process which clients work one-on-one with a trained mental health clinician in a safe, caring, and confidential environment. This forum provides the client an opportunity to receive support and experience personal growth during a challenging time in their life. Individual counseling can help one deal with many personal topics in life such as anger, depression, anxiety, substance abuse, marriage and relationship, parenting problems, school difficulties, career changes, etc.

Social Work Services for Hospice and Home Health Care Providers

Hospice care provides palliative treatment for patients who are in the last stages of a serious illness. Hospice care addresses the patient’s physical, emotional, social and spiritual needs. Social Services is an integral component of the hospice multi-disciplinary approach to addressing the needs of the patient. Social workers help manage emotional distress, provide community resources, and support the wishes of the patient.

Home Health Care is a supportive care provided in the home. Care may be provided by licensed healthcare professionals who provide medical treatment needs or by professional caregivers who provide daily assistance to ensure the activities of daily living (ADLs) are met. Home health services help adults, seniors, and pediatric clients who are recovering after a hospital or facility stay, or need additional support to remain safely at home and avoid unnecessary hospitalization.

We are trained to work collaboratively with your multi-disciplinary team in order to ensure the needs of your patient are met and will ensure your patients are connected to appropriate resources in order to ensure the treatment plan goals of your patients are achieved.

Therapeutic Alliance Professionals provides mental health consultation and training for residential facilities housing members of the community with an intellectual disability. Therapeutic Alliance Professionals provides a field based response to residential facilities in order to ensure the client is having their mental health needs addressed and receiving all available community resources. We will collaborate with staff and review charts in order to address the needs of the client in your facility. By working in partnership with the residential facility, TAP strives to ensure your clients feels supportive while living with their intellectual disability. We provide training for staff in order to ensure they have an understanding of mental illness, developmental disabilities and when is it appropriate to contact law enforcement in order to address clients who poses a danger to themselves or others. These trainings will assist your personnel in successfully addressing the needs of all clients while also ensuring the best interest of all parties are being met.
